We are living in truly extraordinary and unprecedented times. I want to let you know that by speaking with so many of you, my staff and I realize what a difficult and challenging time you may be facing. I want to let you know that you are not alone and my staff and I are here for you. Many of us are facing the same challenges during these stressful times. We know how important it is that you receive your test results as fast as possible. My staff and I are working exhaustively to make this as easy of a process as it can be. It has been extremely difficult to be on the front line of this pandemic as a physician and healthcare provider. Please be kind to one another as everyone can use the kindness. Thank you for entrusting MYCONCIERGEMD with your health! You are now scheduled to have your covid-19 test with our facility.

WHAT TO BRING

1) Download and type your info into this CONSENT, please printout and sign and bring with you to your appointment
2) Your order number ready to show on your phone
3) A pen
4) Drivers License
5) Face mask

INSTRUCTIONS

Please arrive at your scheduled appointment on time. Please note that we have a Beverly Hills testing site and Los Angeles testing site. Please make sure you arrive at the appropriate test site and the location and time you are scheduled at.

Beverly Hills Location – The valet parking area is located off of Rexford drive and Wilshire Blvd. Our address is 9301 Wilshire Blvd.
Please pull past the valet area towards the end of the parking area where you will see two tall red cones and our medical staff.

Los Angeles Location – Located at the parking structure of the former Billingsley’s Resturant

Please be prepared to show your APPROVED ORDER NUMBER to our staff on your phone and let them know which test or tests you are there to do. Please bring a pen with you if you can.
PLEASE AVOID EATING, DRINKING, CHEWING GUM OR USING MOUTH WASH FOR 1 HOUR PRIOR TO THE TEST
We are following the city recommendation for self administered mouth swab vs nasal swab, as the nasal swab is uncomfortable and  we are using a high sensitivity molecular lab to process your specimen. If you would like to do a nasal swab, one of our staff can do it for you. PLEASE WATCH THE VIDEO BELOW TO UNDERSTAND THE PROCESS.
PLEASE have your windows rolled up with you arrive and keep them closed at all times, except when receiving directions from one of our staff.
Our staff will provide you the instructions on how to perform the test.
You will be provided with a CONSENT to read and sign if you did not download prior to your appointment but we highly recommend that you fill it out using ADOBE so is legible and makes the process much faster
You will also be provided with a specimen tube, swab and a collection bag.
Please not that the email placed on the consent will be the email address used to email your results.
Please put your name, date of birth and date of testing on the specimen tube.
You will be asked to bring up your window after the instructions.
You will be asked to cough 3 x forcefully into your mask to bring up any secretions that are present in the back of your throat
You will then remove the cotton swab from the packaging and swab the inside of your mouth, cheeks and back of throat, for at least 30 seconds. Do not touch the swab on any other surfaces.
Place the swab inside the specimen tube and break off the top tip, if you cannot break it off, bend it in, but be sure that the liquid inside does not fall out and the CAP IS SECURELY IN PLACE and TIGHTLY CLOSED.
Our staff will then come and ask you to place the specimen and consent into a collection bag and dropped in a collection tray.
The process should be quick and easy.

If you need to reschedule you results for any reason, please use the login and credentials created. Due to high requests and many who are in need of testing, please reschedule in advance as all slots are booked daily!

HOUSE CALLS

If you scheduled the house call option by adding the + Add Extra Out Call option , please email info@myconciergemd.com letting us know of your request and ORDER NUMBER and confirm address of house call and time, as due to distances we need to verify the time request and make sure we are able to accommodate the exact time requested. There may be delays in regards to staff getting to you at your appointment time due to the volume of requests, so please be leave a 30 minute window.

RESULTS

Your results are typically available within 24 hours. Within 24 to 48 hours of your COVID-19 PCR swab you will receive an encrypted email from info@ultimatedxreport.com with your results.
AT TIMES THE EMAIL SENT FROM THE LAB GOES TO SPAM FOLDER OR TRASH FOLDER OR IS BLOCKED, PLEASE MAKE SURE TO CHECK THESE FOLDERS IN CASE THIS HAPPENS. IF YOU HAVE NOT RECEIVED YOUR RESULTS AFTER 30 HOURS PLEASE EMAIL US AT INFO@MYCONCIERGEMD.COM SO WE CAN LOOK IT UP FOR YOU AND SEND IT TO YOU. Please send email your order number, name, date of birth and day you took the test. Please check your spam or trash for an encrypted email from info@ultimatedxreport.com. Press the read message now button and SIGN IN WITH A WORK OR SCHOOL ACCOUNT to login to retrieve the encrypted message. It will allow you to login with your email. Do not use the one time password.

PLEASE DO NOT CALL US ASKING FOR RESULTS AS THEY WILL NOT BE PROVIDED OVER THE PHONE! PLEASE EMAIL US IF YOU DO NOT RECEIVE RESULTS WITHIN 30 HOURS, PLEASE EMAIL US WITH YOUR ODER NUMBER,  NAME, DATE OF BIRTH, AND SERVICE DATE.
DUE TO OUR LIMITED TIME, WE WILL ONLY CONTACT YOU IF YOUR ANTIBODY TEST RESULTS ARE POSITIVE AND IN BETWEEN SHIFTS OR END OF THE DAY!

WE ARE WORKING EXTREMELY HARD TO PROVIDE THIS SERVICE TO THE COMMUNITY AND GIVEN THE SHEER VOLUME OF REQUESTS AND CALLS, WE ARE HAVING A CHALLENGING TIME GETTING BACK TO EVERYONE IN A TIMELY MANNER. WE KNOW HOW IMPORTANT THIS IS FOR EVERYONE!

**SUNDAY TESTS ARE RUN FIRST THING MONDAY MORNING AND RELEASED MONDAY NIGHT, AT TIMES EARLIER.

RESULTS INTERPERTATION

DETECTED – Means there is active COVID-19 virus detected and you should quarantine for at least 10 days and let anyone else who you have been in contact with know to be tested.
NOT DETECTED – means no viral count was detected.
INDETERMINATE – few results may come up as indeterminate, this is due to the fact that the lab is able to pick up viral material suggestive of an infection but the amount is not high enough to call it a positive or low enough to call it a negative. This is usually due to early infections or late infections. In case you receive an indeterminate result, you should quarantine as this can be an early infection and repeat the test in 5 days.
INVALID – Specimens that do not contain enough genetic material will result in an INVALID test and will be repeated with out an additional fee to you.

COVID-19 ANTIBODY TEST

We will clean your finger tip with an alcohol swab
A small amount of blood will be used for the antibody kits
The needed buffer and solution will be used to perform your test
You will BE NOTIFIED OF POSITIVE results ONLY later In the day.

INSURANCE REIMBURSEMENT

Please let our staff know that you would like to have a super bill to complete and submit to your insurance on the day of your visit.
