House Calls Will Be Provided From 9:00 am to 5:30 pm for additional fee of $450 which covers a 15 mile radius from our office.

To schedule house call option use the Add Extra feature. 

Your credit card billing address must be used for checkout or the order will not process and show ORDER FAILED. Once your ORDER IS APPROVED you will receive an order number, instructions and receipt.  If you are trying to book a house call appointment or booking for someone else, please put the address of the house call in the comment section. Once you register online, you can use your credentials to book for others, please list their names in the comment section with date of birth.

Rapid COVID-19 PCR Swab

Rule out active infection – $225 – We usually provide results in 24 hours or less but it may take up to 48 hours.

Rapid COVID-19 Antibody Test

Rule out previous exposure- Antibodies start to show 5- 7 days after exposure – $225 – same day results

If scheduling a house call instead of a drive through, after your purchase, please email us at notifying us of the house call request, time and address as it may be different than the billing address. Due to distance of house call, there may be a conflict in scheduling that we will try to coordinate with you

Cancellation Policy – Due to the very high demand for appointments and our limited schedule, there are NO REFUNDS. We apologize, we are swamped and just don’t have the time.

Reschedule Policy – Rescheduling is allowed through our online scheduling portal. Please understand that many are looking to book an appointment for testing at our facility and are unable to due to our limited availability. Please be respectful to others and show up to your appointment on time and if you need to reschedule please do it before the day of your appointment.

Insurance & Fee Schedule

Due to high volume are requests, we are unfortunately unable to bill your insurance company on your behalf. Most insurance companies are providing reimbursement for COVID-19 testing. Please ask your insurance carrier in regards to their reimbursement.   Please ask for a super-bill on the day of your test. If you forger to ask for one, please email us with your information, date of service and test you had performed and we will provide you with a super-bill that you can submit to your insurance for reimbursement. We are an out of network provider office. Our usual and customary billing practice is 5 x Medicare rate. The fee for COVID 19 testing performed includes; COVID-19 PCR Test CPT Code U0004, COVID-19 PCR Collection fee CPT Code 62023, PPE Fee and Office Facility Fee, COVID-19 Antibody Fee CPT Code 86328, Currier Fee, consultation fee and other costs associated with being part of our practice and having your test performed at our facility. An Itemized fee breakdown is available upon request.


An encrypted email is sent as soon as results are released by the lab within 24 to 48 hours. There may be delays due to increase in COVID-19 Infections and lab verification of positive results. Due to high volume of calls, we appreciate it if you only EMAIL THE OFFICE at if you have not received your results after 48 hours! We are receiving extremely high call volumes and our staff is having a difficult time answering all the calls. WE WILL NOT PROVIDE YOU RESULTS OVER THE PHONE! WE MUST HAVE YOUR INFO IN EMAIL FROM THE EMAIL ADDRESS PROVIDED TO RELEASE THE RESULTS TO. Please provide your full name, date of birth and date of service. Thank you!
